The 2013 AST Achievement Awards Program is now closed.

The deadline for award nominations was 11:59 PM PACIFIC TIME on MONDAY, NOVEMBER 19, 2012. No nominations or supplementary documents are accepted after the deadline.

AST Clinician of Distinction Award (Non-Physician Award)

Eligibility Criteria:

1. Nominee must be a distinguished non-physician clinician or administrator who is considered an expert in his/her field, whose career is dedicated to transplant and is recognized for outstanding contributions to clinical transplantation.
2. Clinicians eligible include those with a profession within healthcare: Nurses, Transplant Coordinators, Clinical Nurse Specialists, Social Workers, Physician Assistants, Nurse Practitioners, Psychologists, Administrators, Pharmacists, or Dietitians.
3. Nominee's service to the AST will be taken into consideration during the review process.
4. Nominee and nominator must be active members of the AST by the submission deadline. If the nominee/nominator are not members, he/she must submit a completed membership application by the submission deadline (with expectation that membership will be active before the award review occurs). Membership dues must be paid to date by January 1, 2013; otherwise, the nomination will not be considered.

6. No single individual can receive the AST Clinician of Distinction Award within five years receiving a previous AST award.

Application requirements:

1. Nomination application (online)
2. Nominee’s CV uploaded to the application website
3. One (1) letter of recommendation, written and submitted by the nominator. The letter must be uploaded to the application site and the original copy mailed to the AST National Office.

Application Process:
All information will be saved in a secure, password protected environment. At any time in the nomination process, the nominator/submitter will be able to save the nomination and return to the site to complete it at a later date, prior to the deadline.

A nominator must:

• Create and complete a nomination application
• Upload the nominee's CV
• Upload a letter of recommendation and mail the original copy to the AST National Office

The letter of recommendation with original/electronic signatures on institutional letterhead must be uploaded to the application site by the submission deadline. In addition, the original letter must be mailed to the AST National Office. This letter must be postmarked by the submission deadline and have an original signature on institutional letterhead.

Review Process:
The AST Awards and Nominating Committee will review the nominations received by the submission deadline and will make final recommendations to the AST Board of Directors. Nominees and nominators will be notified of the outcome in March 2013. Recipients will receive their award at the AST Awards Ceremony during the 2013 American Transplant Congress, May 18-21, 2013 in Seattle, WA. Recipients are expected to attend the 2013 ATC and must register for the congress. Registration is NOT included as part of the AST award.
