  2. List common transition barriers encountered by pediatric and adult providers.

  3. Identify essential components of transition, including a transition policy and readiness assessment.

  4. Discuss strategies to help the adult provider successfully engage with the newly transitioned young adult transplant recipient.

  5. Discuss the value and challenges of measuring outcomes following transfer from both the pediatric and adult perspective.
