The goals of the AST transition of care initiative are to:
- Advocate and promote effective transition of care (TOC) from pediatric to adult transplant providers for adolescents and young adults (AYA) through a comprehensive and planned approach to care. The process should acknowledge the youth’s individual abilities in skill attainment, family support, and systems issues that affect transition.
- Provide education on all aspects of TOC for the youth, parent/caregiver, and pediatric and adult multidisciplinary teams.
- Engage pediatric and adult transplant teams and the patient voice in developing, strategizing, and implementing a transplant transition program through website resources and peer mentoring.
- Maintain a Transplant Transition Resource Center with links to transition programming, policy development, educational strategies and handouts, mental health resources, and special populations.
