Serial Torque Teno Virus Monitoring to Assess the Net State of Immunosuppression in Kidney Transplant Recipients

Description:

Can a common, benign blood virus help guide immunosuppression in kidney transplant recipients? This session will review a multicenter observational study of 252 kidney transplant patients with longitudinal Torque Teno Virus (TTV) measurements (average 7.8 visits per patient over 2 years). We’ll explore how serial TTV monitoring—considering both current levels and trajectory—can identify patients trending toward subclinical acute rejection or viral infection, supporting more personalized immunosuppression adjustments. A practical “Even‑IS” target range of logTTV 4.5–7.8 will be introduced, where downward slope + low TTV signals higher rejection risk and upward slope signals higher infection risk.

  1. Describe the rationale for using Torque Teno Virus (TTV) viral load as a biomarker of the net immunosuppression state, including the value of serial measurements.
  2. Interpret serial TTV results using both current levels and trends, applying proposed thresholds to stratify patients into under-, even-, and over-immunosuppression risk categories.
  3. Discuss practical considerations for clinical implementation and future validation.
